 Transitional Care Education Class

At some point in your life, your doctor may recommend a short stay in a transitional care setting following an illness or injury.  Transitional care units provide short-term therapy and medical care to help you regain strength and independence, allowing you to return home safety.

Let us answer your questions and help you understand the Transitional Care process.  You will learn more about:

  • Pre-admission
  • What to bring
  • What to expect your first day in TCU
  • What therapy will be like
  • The staff who will be a part of your care team

Transitional Care Educational Classes are held at Minnesota Masonic Home North Ridge on the first and third Wednesday of each month at 10:00 a.m. in the TCU - 8611 55th. Ave. N., New Hope.
