Transitional care is designed for patients who require a short term regimen of therapy usually following a hospital stay. It provides a transition between hospital and home. At our Bloomington transitional care and short-stay rehabilitation facility, we specialize in orthopedic rehabilitation following hip or knee replacements as well as post CVA/stroke rehab. We also provide care for a variety of medical conditions.
